During the online registration process, you will be able to upload the required forms(Birth Certificate, Residency/School attendance record(s) Media Release, Medical Release, etc..)When you complete the Online Registration process. You will see an Order Confirmation.
NOTE: "To complete your registration, please print out and sign the linked Medical Release Form. You will need it to participate this season."
Also, we will send a confirmation to your email. This also will provide a link and now an attachment of the Medical Release Form.
